Benefits are provided through the Health Choice program. Once approved, an enrollment packet will be sent by mail and you may select a physician and a MCO in your area. You may be eligible if your family income is at or below: $16,700 for one person, $22,500 for a family of two or a single pregnant woman, $28,300 for a family of three, and $34,100 for a family of four. Call Healthline at 1-888-561-4090 for an application and more information. Medical Care for Children Partnership (MCCP) also provides medical care to children under the age of 18 through public/private partnerships. Children without health insurance, whose families' incomes are under a percentage of the federal poverty level ($41,000 for a family of four), and who are not eligible for MCHP, may apply.
